The Opportunity: In accordance with the established vision and values of the organization, the Community Licensed Practical Nurse (LPN), in collaboration with the multidisciplinary team, provides care in specified communities and promotes the prevention of disease and maintenance of health and well-being of individuals, families and the community
- Provides holistic, family-centered care across treatment settings to improve the overall health and well-being of individuals, families and the community.
- Provides personal care to home and community care patients in accordance with home support/LTC personal assistance guidelines as well as care plans and homecare assessments.
- Maintains up to date patient medical records to ensure that all patient issues are accurately and clearly stated and changes are reflected as they occur. In addition, ensures their charting is accurate, complete and in compliance with regulations, certification standards, legal and ethical standards.
- Recognizes and interprets symptoms and institutes measures within the scope/limits of a Licensed Practical Nurse in BC and reports significant changes in the patient condition to CSFS Primary Health Care Team.
- Administers medication and treatments as ordered by the physician, including oral, subcutaneous and intramuscular medications in accordance with LPN scope of practice, pharmaceutical and organizational policies and procedures.

What You Will Bring to The Role:
- Graduate of an accredited school of practical nursing.
- Current BC LPN license.
- First Aid Level 1

Wage Range: The target hiring range for this position is $34.0759 to $44.1058 per hour. Candidates will typically be hired, transferred or promoted between the minimum and midpoint of the wage range. Placement within the wage range is dependent on the applicants’ role specific knowledge, skills, abilities, related experience and in consideration of factors such as market conditions and internal equity.
