Family Nurse Practitioner

A well-established nonprofit healthcare organization serving the San Jose community is seeking an experienced Family Nurse Practitioner to join its outpatient primary care team. This full-time position offers the opportunity to provide comprehensive care to a diverse patient population in a collaborative, mission-driven environment focused on expanding access to quality healthcare for underserved communities.

The provider will manage an established patient panel across the lifespan, from pediatrics through geriatrics, while working alongside physicians, nurse practitioners, behavioral health professionals, nurses, and medical assistants in an integrated care model. This is an autonomous role ideal for an experienced NP who enjoys building long-term patient relationships and practicing independently with strong team support.

Position Highlights

• Full-time outpatient position
• Monday through Friday schedule
• Standard business hours with rotating late shift on Thursdays
• Minimum commitment of 32 hours per week
• Patient volume of approximately 15–20 patients per day
• Care for patients across the lifespan, including pediatrics and geriatrics

• No hospital call responsibilities

Compensation & Benefits

• Salary range of $150,000–$170,000 annually based on experience

Requirements

• Family Nurse Practitioner certification
• Active California NP license
• Minimum of 2–3 years of primary care experience
• Ability to practice independently
• Experience working with diverse or underserved populations preferred
• EPIC experience preferred
• COVID vaccination compliance required

Pay: $150,000.00 - $175,000.00 per year
