About the Role
The Family Nurse Practitioner will provide comprehensive primary care services to a diverse patient population in a modern outpatient setting. The practice prioritizes access, preventive care, and long-term patient relationships while offering strong clinical and administrative support.
Responsibilities

Provide outpatient primary care services to patients across the lifespan

Evaluate, diagnose, and manage acute and chronic medical conditions

Perform routine exams, preventive visits, and health screenings

Order and interpret diagnostic tests and imaging

Prescribe medications and develop evidence-based treatment plans

Educate patients on health maintenance, disease prevention, and lifestyle management

Maintain timely and accurate documentation within the electronic medical record (EMR)

Collaborate with physicians, specialists, and clinical support staff to ensure coordinated care
Requirements
-
Active Family Nurse Practitioner (FNP) certification

Current or pending California Nurse Practitioner license
-
Prior outpatient or primary care experience preferred (new graduates may be considered)

Strong clinical judgment and patient-centered care approach

Excellent communication and documentation skills
