About the Role

The Family Nurse Practitioner will provide comprehensive primary care services to a diverse patient population in a modern outpatient setting. The practice prioritizes access, preventive care, and long-term patient relationships while offering strong clinical and administrative support.

Responsibilities

  • Provide outpatient primary care services to patients across the lifespan

  • Evaluate, diagnose, and manage acute and chronic medical conditions

  • Perform routine exams, preventive visits, and health screenings

  • Order and interpret diagnostic tests and imaging

  • Prescribe medications and develop evidence-based treatment plans

  • Educate patients on health maintenance, disease prevention, and lifestyle management

  • Maintain timely and accurate documentation within the electronic medical record (EMR)

  • Collaborate with physicians, specialists, and clinical support staff to ensure coordinated care

Requirements

  • Active Family Nurse Practitioner (FNP) certification

  • Current or pending California Nurse Practitioner license

  • Prior outpatient or primary care experience preferred (new graduates may be considered)

  • Strong clinical judgment and patient-centered care approach

  • Excellent communication and documentation skills
