Medical Care Coordinator (Remote)

<p>We’re seeking a <strong>Medical Care Coordinator</strong> to support members as they navigate their healthcare needs, benefits, and available resources. This role is ideal for someone who is patient‑focused, organized, and passionate about improving access to care.</p><p><br></p><p><strong>What You’ll Do</strong></p><ul><li>Serve as the first point of contact for members in care management</li><li>Help members understand benefits, coverage, and care options</li><li>Conduct proactive outreach to address gaps in care and prior authorization needs</li><li>Provide disease‑specific education (adult & pediatric) under a licensed Care Manager</li><li>Identify social determinants of health and help remove barriers</li><li>Manage a caseload of 150–300 members with stable but complex or rising‑risk conditions</li></ul><p><br></p><p><strong>What You Bring</strong></p><ul><li>Background in Social Work or Nursing preferred</li><li>Associate’s degree in a healthcare field or LPN required</li><li>Schedule: Monday–Friday, 8:00 a.m.–4:30 p.m.</li></ul><p><br></p><p>If you’re passionate about guiding members through their healthcare journey and supporting better outcomes, this role offers a meaningful opportunity to make an impact.</p><p><br></p><p><em>HirePower Personnel, Inc. is an Equal Opportunity Employer.
